For Tier 1 and Tier 2 tickets please submit the following information.
  1. Ticket Number
  2. User ID
  3. First Name, Last Name
  4. Support follow contact
  5. Mobile Device Make and Model
  6. Device Operating System and Version
  7. Cureatr Version (can be found in Settings)
  8. Connectivity (mobile data or Wi-Fi)
  9. If Wi-Fi, is Wi-Fi Network public or private?
  10. Is failure repeatable by support?
  11. Steps taken prior to failure point.
